This project will create a program that increases awareness and knowledge of “brain foods” and sustainable agriculture within the community of Oakville. These services will be expanded to address the needs of those at an elevated risk and who are disenfranchised by these social determinants of health. Through teaching, rehabilitation and education, risk levels are mitigated; in turn, preventing poor nutritional choices while increasing overall awareness.

 

In addition, this program will provide the tools and supports to assist our clients and their families in maintaining their place in the community to the greatest degree.

 

Mind Forward Brain Injury Services (previously Peel Halton Dufferin Acquired Brain Injury Services) provides community based rehabilitation to survivors of acquired brain injury (ABI). ABI is damage to the brain that occurs after birth from a traumatic or non-traumatic event.
